Anatomy of a Concussion

It seems that you can't watch SportsCenter without hearing about an athlete affected by head injury. Whether it is the hockey player out three weeks after being checked into the boards, or the quarterback considering retirement from repeated concussions, head injuries are prevalent in the sports world. But not just elite athletes are affected. The Centers for Disease Control estimate that over 300,000 people per year suffer significant head injuries participating in sports. Half of all those will suffer some kind of temporary impairment and six percent will suffer permanent damage. Concussions are often referred to as a “silent epidemic” because they often go unreported and untreated so the actual number of injuries is likely much higher

The Phases of a Head Injury

Sports-related traumatic brain injury (SRTBI) begins with some force being applied to the head. Typically, this force involves rapid deceleration of the head, either from a collision or whiplash type event. This can cause bleeding, stretching, and sheering of brain tissue and other mechanical damage. But for most, this is only the beginning. After the initial injury, in which necrotic cell death occurs from the initial insult, chemical changes in the brain lead to a second phase of cell death called apoptosis. In the secondary phase of a SRTBI a cascade of changes in the brain occur resulting in the death of neuronal cells. This process is called the excitotoxic cascade. This excitotoxic cascade is chemical rather than physical. Damage can occur hours or even days after the initial injury. The cell death in this phase is often referred to as programmed cell death or cell suicide. In this process cells that are less injured than those that have already died immediately begin an adaptive response by repairing the injured elements within the cell or if they are more severely injured, the active elimination of cells that are presumably too damaged to be repaired. Cells, in their attempt to recover, purge severely injured cells causing a second and in some cases, larger wave of brain cell die off. While little can be done about the initial mechanical trauma and cell death, the chemical changes and resultant damage from the excitotoxic cascade can be ameliorated by chemical neuroprotection.

Symptoms of a SRTBI can be mild, moderate, or severe, depending on the extent of damage to the brain. Mild cases may result in a brief change in mental state or consciousness, while severe cases may result in extended periods of unconsciousness, coma, or even death.

After the Fact

Unfortunately, despite extensive research and continued efforts by the emergency medical community there is little that can be done to reduce the effects of SRTBI after the injury has been suffered.
